EHS FAMILY SERVICE WORKER COORD. – Early Childhood Ed. –Full time

BASIC FUNCTIONS:         Provide support and coordination of services for children

                                               and families enrolled in the EHS Program.

SUPERVISOR:                    Director of Early Childhood Education

FLSA:                                    Hourly – Non-exempt

POSITION RESPONSIBILITIES:

1.                  Recruit children/families and pregnant women for home-based and center-based Early Head Start services.

2.                  Develop and maintain relationships with local organizations serving Early Head Start eligible children, families and/or pregnant women.

3.                  Conduct initial screening/application for families to determine eligibility for services.

4.                  Act as Coordinator for health and mental health services for home-based and center-based children and families.

5.                  Responsible for conducting all parent committee meetings and events.

6.                  Submit reports of all parent committee meetings and events.

7.                  Maintain accurate updated case files.

8.                  Compile reports and statistical data as required or as requested by supervisor.

9.                  Coordinate family orientation to services of La Casa de Esperanza.

10.              Assess family needs.  Develop strategies to address identified needs through programs, services and other resources at La Casa and in the community.

11.              Oversee case management and the provision of other direct services (i.e., counseling, medical care, etc.).

12.              Identify home-based and site-based services to address strengths, interests and needs of families.  Establish mutual goals and plans for achieving desired outcomes.

13.              Develop family partnership agreement with each family on caseload and review quarterly.

14.              Conduct home visits in accordance with La Casa policies and EHS performance standards (more frequently if needed).

15.              Maintain communication with EHS staff, parents and community organizations.

16.              Coordinate Health Services Advisory Committee (quarterly meeting)

SKILLS AND QUALIFICATIONS:

  1. 1-3 years professional experience in service coordination counseling, early childhood education or childhood development.
  2. Shall be at least 18 years of age.
  3. Diplomacy and human relations skills.
  4. Knowledge of or ability to quickly learn about services for pregnant women, infants, toddlers and their families.
  5. Must be reliable and flexible to meet the needs of the program.
  6. Ability to work as a team member.
  7. Ability to work effectively with diverse families.
  8. Ability to work in a fast paced environment.
  9. Excellent written and oral communications skills.
